A little progress.
A family plan.
Build a weekly behavior chart around actions your family wants to practice. Choose clear goals, decide how to recognize them, and check in together.
Your chart stays in this page. It is not saved or sent to us. Print or download it before leaving.

Make the plan together
Start with a small number of actions that fit the person's abilities, routine, and available support. Write down what completion looks like. Recognition can be a specific thank-you or a shared activity; payment is optional. Adults can model the same everyday responsibilities without using child reward arrangements.
The American Academy of Pediatrics' reward guidance describes charts built around clear actions and agreed rewards. Our chart is an original planning resource, not an evaluated treatment or a promise of behavior change.
If your family chooses points, write the agreed arrangement in the recognition field. This chart records completed actions; it does not total points or calculate a balance. The AAP reward guidance concerns school-age children ages 5 to 12, so choose a plan that fits the child rather than applying the same points system to everyone.
Use progress to start a conversation
At your check-in, talk about the support that helped and whether the plan was realistic. Keep affection, food, rest, and other basic needs outside reward agreements. Use this chart for positive recognition; it does not subtract progress or calculate penalties.
For examples you can adapt, read positive reinforcement examples for family life. For household task scheduling, use the family chore chart.
